  • Why don't you just fill out the blanks in Word and print it? I don't see a need to recreate this in any way. Commented Jan 7, 2019 at 9:28
  • Something you need: the font (that's maybe Helvetica, I think), the margin setting by geometry package, something to add a line from the left edge of the page to the right edge, and maybe some tables... Commented Jan 7, 2019 at 9:33
  • @JouleV I doubt the dotted lines are needed in print if the document is filled out electronically. People need it when they fill out the paper with a pen. Commented Jan 7, 2019 at 9:41
