1Freighter travel, if it's available, generally requires a medical certificate, as you'd be far from any health care services for an extended period of time. 3If you do need to ask for any accommodations from the airline, then they're most likely going to want a letter from your doctor, for two reasons: to verify that accommodations are needed, and to verify that it is safe for you to fly at all. The airline also has their own doctors who will read this and make their own decision. If they're not convinced that it is safe for you to fly, they can and will refuse to transport you. 1Airports/airlines have procedures for passengers with special needs and at least in Europe you can request those from the airline without additional costs.
