  • The updated ASTRO clinical practice guideline for pancreatic cancer reflects growing evidence for radiation therapy across the disease spectrum, from resectable and locally advanced to oligometastatic settings. Dr. Michael Chuong, vice chair of the guideline task force, highlights how MR-guided and online adaptive radiotherapy are enabling more comprehensive treatment, including coverage of regions at risk for microscopic disease and dose escalation to gross tumor, with the goal of durable local control for appropriately selected patients.   • What if imaging could show which metastases will respond to treatment, and which need more? Dr. Nikolai Strusberg shared findings at American Society for Radiation Oncology (ASTRO) #ASTRO26 from a lesion-level, AI-driven PSMA PET/CT analysis in patients treated with Lu-PSMA therapy for metastatic castration-resistant prostate cancer. Baseline PET features were associated with how individual lesions responded, supporting quantitative PSMA PET as a potential tool for response-adaptive strategies, including future trials that pair radiopharmaceutical therapy with targeted external beam radiation.

  • Reirradiation is often limited to palliative doses due to concerns about cumulative toxicity to nearby organs. At American Society for Radiation Oncology (ASTRO) #ASTRO26, Dr. Kathryn Mittauer presented cumulative GI dose findings from our early prospective trial of ablative, 5-fraction MR-guided reirradiation. In 8 of 10 patients, cumulative doses to GI organs at risk exceeded thresholds from the recent ReCOG systematic review, with no grade 3 or higher toxicity observed. Daily on-table adaptive radiotherapy with MR visualization was key to safely escalating dose in this setting.

  • Proton therapy is increasingly used to treat localized prostate cancer, and long-term data help guide how it's used for different patients. At #ASTRO26, Dr. Marcio Fagundes shares results from his study of 404 patients treated with intensity-modulated proton therapy. With a median follow-up of more than four years, men with intermediate-risk disease had very rare recurrences. Patients with high-and very-high-risk disease had a few more, as expected.   • Radiation therapy is an important tool in treating brain tumors, and managing its potential side effects is part of delivering thoughtful care. At American Society for Radiation Oncology (ASTRO) #ASTRO26 Dr. Rupesh Kotecha, discussed contemporary approaches to diagnosing and managing radiation necrosis. He walked through its four grades, ranging from changes seen only on MRI to cases needing steroids, escalated therapy, or minimally invasive and invasive strategies. His presentation focused on the latest evidence-based strategies to help guide treatment for these patients.
