We’re incredibly excited to announce that Counsel is joining the CMS Innovation Center’s ACCESS Model, bringing AI-native chronic care to Medicare beneficiaries at no out-of-pocket cost. 🩺 Starting in early 2027, eligible Medicare beneficiaries with hypertension, obesity, hyperlipidemia, or prediabetes will have access to Counsel’s modern, connected care experience. Patients will receive personalized guidance from Counsel’s medical AI alongside on-demand access to our board-certified physicians. We’re also expanding our partnership with ŌURA. For eligible Oura Members ages 65+, Counsel will incorporate continuous biometric data into how care is delivered, helping members move seamlessly from health insights to clinical action within the Oura App. More than 90% of adults 65+ live with at least one chronic condition, yet many advances in digital health have not reached Medicare beneficiaries at the same scale. According to the ONC, healthcare organizations only have access to approximately 43% of a patient's data from outside their own network, contributing to growing care gaps. When patient context is fragmented, care becomes reactive. Opportunities for health plans to intervene earlier and guide members toward the right setting are often missed, creating unnecessary costs and downstream utilization. Even when clinical data is available at the point of care, it's rarely put to full use, leaving physicians to make decisions without the complete picture.
