  • Clinical outcome: Hampden Post Acute OrthoWIN Rehabilitation Supports Measurable Recovery After Femur Surgery A 61-year-old female arrived at Hampden Post Acute following a mechanical fall that resulted in a left distal femur intra-articular fracture. After undergoing open reduction and internal fixation of the left femur, she transitioned to Hampden for clinical optimization and OrthoWIN orthopedic-focused rehabilitation. Her individualized plan combined consistent physical and occupational therapy, a customized pain-management program with onsite physiatry support, wound care, vital-sign and weight monitoring, in-house laboratory testing, fall-prevention education, and psychosocial support through counseling and talk-therapy sessions. At admission, she was completely dependent for toileting and required maximum assistance with lower-body dressing, moderate assistance with bathing and bed mobility, minimum assistance with transfers, and supervision with grooming and upper-body dressing. By discharge, she could complete grooming independently. She achieved modified independence with dressing, toileting, bed mobility, and transfers and required only setup assistance with bathing. Her Model 10 composite functional outcome score improved from 3.9 at admission to 9.0 at discharge. After a successful 20-day rehabilitation stay, she returned home with services through Excel VNA and follow-up appointments arranged with her primary care and orthopedic providers. This outcome reflects the value of coordinated, whole-person orthopedic rehabilitation in helping patients progress from substantial care needs toward a safe and supported return home.
