Upward Health is an in-home medical group that specializes in primary medical, behavioral, and social healthcare for individuals with complex needs. We deliver care in the community. We reduce barriers to care, such as long delays due to scheduling or lengthy commutes to a provider’s office by meeting the patient right at home. We see patients when they need us, for as much time as they need. Beyond medical and behavioral care, we also assist our patients with challenges that may affect their health, such as food insecurity, social isolation, housing needs, transportation and more.

  • Trust and connection are at the heart of whole-person care. We’re grateful to have Dr. Elizabeth Bogel Ryan sharing this important perspective and reinforcing the importance of meeting patients where they are.

    Very grateful for the opportunity to reconnect with my former med school classmate Elizabeth Bogel Ryan - Family and Addiction Medicine Physician and Executive Consultant at Upward Health - and learn from her experience during this month's Grand Rounds, aptly titled "Good Addiction Care is Good Human-ing." Dr. Ryan shared principles of evidence-based treatment for opioid and alcohol use disorders while emphasizing a powerful truth: successful SUD care is built on building trust through strong communication and reliable follow-through.  How we listen to our patients and respond to their unique needs as a whole person has a major impact on our ability to engage them in care. Thank you, Dr. Ryan, for reminding us that great healthcare starts with trust and connection.

    Upward Health is heading to Washington, D.C. for the ACAP CEO Summit 2026 on October 1–2. We’re excited to have Glen Moller, Chief Executive Officer, and Seth Zuckerman, Chief Business Officer, representing Upward Health at this year’s summit. The Association for Community Affiliated Plans (ACAP) CEO Summit brings together leaders from community health plans and the broader health care community to connect, exchange ideas, and discuss the issues shaping the future of care for Medicaid and other underserved populations. As health plans continue to navigate a rapidly changing healthcare landscape, conversations around whole-person care, access, and better outcomes for high-need communities are more important than ever. Glen and Seth look forward to connecting with fellow health plan leaders, sharing perspectives, and exploring opportunities to work together to improve care for the communities we serve.

    Bringing California leaders together! Some of California’s West Coast Supervisors of Care Coordination and Managers recently came together for a day focused on collaboration, connection, and strengthening our work across markets. The meeting included important ECM updates, operational refreshers, organizational updates, and opportunities to share insights and align across teams. It was also a great opportunity for team bonding and relationship-building among leaders from across the state.   • During Suicide Prevention Week, we’re reminded that sometimes, simply being there can make all the difference. In this patient testimonial, Raina shares what it was like to experience a very difficult period in her life while struggling with her mental health. At a time when she felt like she had reached a very low point, her Care Specialist, Nancy Rowemanns, showed up for her when she needed someone most. Nancy had never met Raina before that first interaction, but she didn’t hesitate to be there for her during a moment of distress. For Raina, that support became a source of hope, and she describes Nancy as the “light at the end of the tunnel.” This is what compassionate, person-centered care can look like: meeting someone where they are, listening, and reminding them that they don’t have to face difficult moments alone.
