Departmental Forms Management

Welcome to the Departmental Forms Management Web page.  You will find links to the Departmental forms, bureau/office Web sites, other forms Web sites, and bureau/office forms contacts.

If you have Trust Beneficiary questions, please call the Trust Beneficiary Call Center (TBCC) at 1-888-678-6836.  Hours of Operation are Monday through Friday, 7 a.m. to 6 p.m. and Saturday 8 a.m. to Noon (Mountain Time).

If you need to create a new form, update an existing form, or have a general forms-related question, please contact the respective bureau/office forms contact below to begin the forms process. 

All electronic forms must be compliant with Section 508 of the Rehabilitation Act, as amended.  All new and updated forms, regardless of format, require consultation with all seven stakeholders below in your respective bureau/office to ensure compliance with various laws, regulations, and policies.  

All forms also need to comply with Departmental Manual 387 – Chapter 1, Departmental Forms Management Master Style Guide, and Departmental Forms Management Development Process Guide. Please contact your respective bureau/office forms contact for these documents.
