A lot of the conversation around digital health tends to swing between extremes: either it’s going to fix everything, or it’s overwhelming clinicians. This #Medscape commentary authored by Dr. Nelly Abulata offers a much more grounded perspective: digital care is neither. It’s a workflow problem. She proposes a simple but powerful lens for evaluating any digital tool: Does it remove friction… or just move it somewhere else? Walking through a typical clinic day, the piece highlights where that friction often manifests. Before the visit, digital intake and triage tools can surface useful insights...but they can also generate noise, increasing cognitive load as HCPs sort through alerts and flags that may not be actionable. During the visit, tools like ambient listening / documentation can improve efficiency and reduce administrative burden. But they don’t eliminate responsibility; they shift it. HCPs still need to validate, interpret, and correct what’s generated. And when trust in the record is low, speed quickly turns into rework. After the visit, digital follow-up, messaging, and remote monitoring can improve continuity of care. But without clear workflows, they can also create an “always-on” clinical environment, where message volume rises and boundaries between visits blur. The theme is thus: Technology is never experienced in isolation; it’s experienced as workflow. For those of us thinking about digital transformation in healthcare, this is a helpful reminder. Adoption metrics alone don’t tell the story. What matters is cognitive load, trust in the data, and who ultimately absorbs the extra work. A practical takeaway from the piece: evaluate digital tools the same way you would a clinical intervention -- not just by what they promise, but by what new work they create, and where that work lands. The Johari Window: A Leadership Lesson for CMOs I recently heard an observation about a physician executive relatively new to the CMO role: “They don’t know what they don’t know.” It stayed with me. It can sound like criticism, but it describes one of the most important challenges of stepping into senior leadership. New CMOs bring considerable expertise. They understand medicine, have led physicians, and have solved difficult clinical problems. Then the role suddenly becomes much broader: quality, patient safety, regulatory, medical staff governance, graduate medical education, hospital-based physicians, and hospital operations that often requires to influence outcomes through people and processes you don't directly control. The challenge isn't simply learning what you don't know. It's discovering what you don't yet know that you need to know. That reminded me of the Johari Window, a framework for self-awareness. Two of its quadrants are particularly relevant to new leaders: The Blind Spot: what others see that you don't. As our titles become more senior, people can become more careful about what they tell us. We may receive more information but less candor. The Unknown: what neither you nor others have discovered yet. A new role inevitably presents situations you've never encountered. That can look like a serious safety event, a difficult peer-review decision, a regulatory crisis, or the need to influence an outcome without direct authority. The answer isn't to try to know everything. It's to become exceptionally good at reducing your unknowns. Ask questions before offering answers. Build a personal board of advisors who will tell you what you need to hear, not simply what you want to hear. Instead of asking, “Do you have any feedback?” ask, “What am I missing?” Spend time where the work happens. Dashboards tell us what happened. The ED, bedside, OR, physician lounge, and nursing unit often tell us why. And get comfortable saying, “I don't know - but I know who we need to bring into the conversation.” Most importantly, create enough psychological safety that people are willing to tell you when you're wrong. The best leaders aren't necessarily those who know the most. They are the ones who create relationships, feedback loops, and habits that make it difficult not to learn. None of us knows what we don't know. The difference is how deliberately we go looking for it.

88% of Americans think healthcare is becoming less affordable. Most assume it's a pricing problem. It's not. It's a design problem. I sat down with Adam Siskind from ZS Associates, who surveyed 12,000 patients and 1,500 providers across 7 countries. What he found is hard to ignore. CMS is the largest payer in the world — yet it doesn't exert the purchasing leverage that other governments do. Other countries make explicit trade-offs between cost and quality. In the U.S., that question stays politically charged instead of operationally resolved. Affordability trends aren't random. They reflect deliberate system design choices. COVID didn't create the crisis. It just stress-tested choices that were already baked in. That's not a market failure. It's a structural consequence. 🎧 Full episode linked in comments.
