So I taught myself how to run the analysis (and ran it) on ICU data from a major academic medical center. The question: when a routine nursing check — is the patient oriented, do they know where they are — doesn't get documented on schedule, what happens to that patient? 𝗠𝗼𝗿𝗲 𝘁𝗵𝗮𝗻 𝗱𝗼𝘂𝗯𝗹𝗲 𝘁𝗵𝗲 𝗼𝗱𝗱𝘀 𝗼𝗳 𝗱𝘆𝗶𝗻𝗴 𝗶𝗻 𝘁𝗵𝗲 𝗵𝗼𝘀𝗽𝗶𝘁𝗮𝗹 — after accounting for how sick the patient already was, their age, everything else they had. Not the result of the check. Its absence. Then a thing that surprised me. In a second analysis across over 45,000 admissions (submitted to BMJ Quality & Safety journal), the signal nearly doubled at night. Weekends — were fine — changed nothing. Staffing ratios don't explain that pattern. Cognitive load does. Any experienced pilot already knows this. Three analyses under peer review now. The signal keeps showing up. The record doesn't just tell you what happened to a patient. 𝘛𝘩𝘦 𝘨𝘢𝘱𝘴 𝘵𝘦𝘭𝘭 𝘺𝘰𝘶 𝘸𝘩𝘢𝘵'𝘴 𝘢𝘣𝘰𝘶𝘵 𝘵𝘰.

The Aviation Safety Reporting System lets a pilot confidentially report his own error in exchange for immunity. The mistake enters the system's memory instead of being buried. The entire point is to catch the problem upstream — before it becomes a smoking hole. 𝗠𝗲𝗱𝗶𝗰𝗶𝗻𝗲 𝗺𝗼𝘀𝘁𝗹𝘆 𝗰𝗼𝘂𝗻𝘁𝘀 𝗵𝗮𝗿𝗺 𝗮𝗳𝘁𝗲𝗿 𝗶𝘁 𝗹𝗮𝗻𝗱𝘀. Morbidity and mortality conferences behind closed doors. Incident reports that go nowhere. Malpractice discovery years later. Nasim Afsar, MD MBA MHM wrote an incredible book called Intelligent Health regarding similar challenges. The near miss — the thing that almost happened, the check that almost got skipped — usually leaves no trace at all. 𝙔𝙤𝙪 𝙘𝙖𝙣𝙣𝙤𝙩 𝙚𝙣𝙜𝙞𝙣𝙚𝙚𝙧 𝙖𝙧𝙤𝙪𝙣𝙙 𝙖 𝙙𝙖𝙣𝙜𝙚𝙧 𝙮𝙤𝙪 𝙣𝙚𝙫𝙚𝙧 𝙧𝙚𝙘𝙤𝙧𝙙𝙚𝙙.

An independent body with no stake in the outcome — the NTSB — tears it apart. Its findings become federal requirements every operator must follow. Not suggestions. Grounding is the penalty. A pilot cannot fly the approach his own way. Unexplained variation is treated as a hazard. 𝗠𝗲𝗱𝗶𝗰𝗶𝗻𝗲 𝗵𝗮𝘀 𝗻𝗼 𝗲𝗾𝘂𝗶𝘃𝗮𝗹𝗲𝗻𝘁 𝗹𝗼𝗼𝗽. Guidelines are recommendations. Variation is professional judgment. And there is no single authority that turns "what went wrong" into "what everyone must now do" — findings scatter across journals, committees, and fifty state boards, and enforcement is nobody's job. Some of that is structural — one FAA versus a fragmented system is not a fixable choice. But treating unexplained variation as a hazard instead of a prerogative? 𝙏𝙝𝙖𝙩 𝙤𝙣𝙚'𝙨 𝙖 𝙘𝙝𝙤𝙞𝙘𝙚.

𝗠𝗲𝗱𝗶𝗰𝗶𝗻𝗲 𝗻𝗲𝘃𝗲𝗿 𝗯𝗲𝗻𝘁 𝗶𝘁𝘀 𝗰𝘂𝗿𝘃𝗲. 1968–1977: 1 fatality per 350,000 boardings. 2018–2022: 1 per 13.7 million. Over the same half-century, diagnostic error alone is associated with an estimated 795,000 Americans killed or permanently disabled every year — and that's the floor, only what gets measured (𝘕𝘦𝘸𝘮𝘢𝘯-𝘛𝘰𝘬𝘦𝘳 𝘦𝘵 𝘢𝘭., 𝘉𝘔𝘑 𝘘𝘶𝘢𝘭𝘪𝘵𝘺 & 𝘚𝘢𝘧𝘦𝘵𝘺, 2024). Two complex, high-consequence, expert-driven fields. One drove its harm rate down relentlessly. The other did not. 𝗔𝗻𝗱 𝗶𝘁'𝘀 𝗻𝗼𝘁 𝘁𝗵𝗲 𝗽𝗲𝗼𝗽𝗹𝗲. Medicine's people are not less capable or less committed than aviation's — by most measures the opposite. The interesting question isn't whether medicine should learn from aviation. Everyone says that, usually as "we need more checklists." The question is why the two diverged — which differences are structural and immovable, and 𝙬𝙝𝙞𝙘𝙝 𝙖𝙧𝙚 𝙘𝙝𝙤𝙞𝙘𝙚𝙨 𝙩𝙝𝙖𝙩 𝙘𝙤𝙪𝙡𝙙 𝙨𝙩𝙞𝙡𝙡 𝙗𝙚 𝙢𝙖𝙙𝙚 𝙙𝙞𝙛𝙛𝙚𝙧𝙚𝙣𝙩𝙡𝙮.

In 1918, the U.S. Post Office hired civilians to fly the mail. Open-cockpit biplanes. No radios, no instruments, no navigation aids — landmarks and dead reckoning, maps strapped to their legs. 𝗧𝗵𝗲 𝗮𝘃𝗲𝗿𝗮𝗴𝗲 𝗺𝗮𝗶𝗹 𝗽𝗶𝗹𝗼𝘁 𝗵𝗮𝗱 𝗮 𝗹𝗶𝗳𝗲 𝗲𝘅𝗽𝗲𝗰𝘁𝗮𝗻𝗰𝘆 𝗼𝗳 𝗮𝗯𝗼𝘂𝘁 𝟵𝟬𝟬 𝗳𝗹𝘆𝗶𝗻𝗴 𝗵𝗼𝘂𝗿𝘀. Of the first 40 hired, three were killed in 1919. Nine more in 1920. Then the machinery arrived. Lighted airways, beacons, instruments, weather reporting. In 1919, one pilot died for every 115,000 miles flown. By 1926, one per 2.58 million — a 22–𝘧𝘰𝘭𝘥 𝘪𝘮𝘱𝘳𝘰𝘷𝘦𝘮𝘦𝘯𝘵 in seven years. Nothing about flight became less dangerous. 𝙏𝙝𝙚 𝙙𝙖𝙣𝙜𝙚𝙧 𝙬𝙖𝙨 𝙚𝙣𝙜𝙞𝙣𝙚𝙚𝙧𝙚𝙙 𝙖𝙧𝙤𝙪𝙣𝙙.

This is a great evaluation of the direct contracting revolution. Employers often don't see the many layers of intermediaries that are all taking a slice out of their benefits costs - they are not required to be reported transparently. Going direct will change the lens of who's hands are in the mix, between your employees and the care you want to ensure they receive. There are far more than one might think!
