Thanks for sharing this important message and the thorough calculator. I hear this all the time about retainers and 1 other reason you didn't mention... "I need to keep my staff busy." The reality is that we orthodontists outsource every day - we don't make our own dental chairs, we don't make our own toothbrushes... if a service provider/ company can do at least 2 of the following 5 categories better, you should seriously consider it. Cost, Quality, Time, Volume, and Experience.

If I want to summarize the feedback we received at AAHKS, I would say: Monogram has surfed to the other side of the uncanny valley of surgical robotics. It is safe, accurate, intuitive, easy to use, and fast! I recently read a post that seemed more emotional than rational. Here is my honest roboticist prospective on a few topics raised in that post: Would robots take the surgeons’ jobs? Way after all the other jobs are replaced by robots, considering the criticality and delicacy of a surgeon’s tasks. Would surgeons’ revenue be diverted to robots? No, as long as the liability is owned by the surgeon. A robot and a surgical knife or a manual saw are not that different. They are simply tools. Where does the money for robots come from? A well-designed robot would increase the surgeon’s volume through reduction in fatigue and stress and also surgical time. It also improves patients’s satisfaction leading to increased revenue. ��We don’t even know where it tells the robot to put the implants”? Behold the screen “Make yourself sheep and the wolves will eat you”: Sorry, I didn’t understand this part.

Capability defines liability. At scale, do we really need more surgical robotic arms, AI dashboards, multi-modality imaging integration, connected care platforms that tease digital ecosystems? The current pile-up in the large format sector of robotics (see below from Oppenheimer robotics report) at the door of yesterday’s definition of performance and design for a fixed site of care is likely setting up for disappointment. The outdated target capabilities in a majority of these systems are tied up in 10-year R&D pipelines, anchored to a legacy site of care, and performance metrics that focused on margins, market share, and over-featured systems. The fixation of the pursuit of the Intuitive platform, form-factor, and hyper-featured soft tissue robotic platforms in the U.S. market by the strategics is already a well-baked and hyper-dominated market that will pull these delusional organizations into the deep end of the surgical robotic pool to struggle mightily while missing the U.S. based ASC market and the lower cost yet higher volume OUS emerging markets. The current form factors by the strategics are likely going to be too expensive, too complex, too much training required, and not enough resources to deploy to serve the current legacy market of hospitals while battling the incumbent who has a stranglehold on the model. What company or format will properly address the greenfield market environment of “good enough”, lower cost, faster setup & breakdown, shared risk model of both ASC’s as well as the alternative to Tier 1 hospital settings which is the real rapidly expanding market? The strategics install base and business models tied to hardware economics can get trapped in yesterday’s definition of performance. The stronger the capability, the bigger the inertia, the more difficult to abandon. Be bold enough to put yesterday’s version of you out of business before someone else does. Because they will.
