  • Back pain in space and post-flight spine injury: Mechanisms and countermeasure development

    The pathogenesis of lumbar back pain during spaceflight is most likely discogenic and somatic referred (from the sinuvertebral nerves) due to supra-physiologic swelling of the lumbar intervertebral discs (IVDs) due to removal of gravitational compressive loads in microgravity. The knee-to-chest position may reduce lumbar back pain by redistributing stresses through compressive loading to the IVDs, possibly reducing disc volume by fluid outflow across IVD endplates.     The pathogenesis of lumbar back pain during spaceflight is most likely discogenic and somatic referred (from the sinuvertebral nerves) due to supra-physiologic swelling of the lumbar intervertebral discs (IVDs) due to removal of gravitational compressive loads in microgravity. The knee-to-chest position may reduce lumbar back pain by redistributing stresses through compressive loading to the IVDs, possibly reducing disc volume by fluid outflow across IVD endplates. IVD stress redistribution may reduce Type IV mechanoreceptor nerve impulse propagation in the annulus fibrosus and vertebral endplate resulting in centrally mediated pain inhibition during spinal flexion. Countermeasures for lumbar back pain may include in-flight use of: (1) an axial compression harness to prevent excessive IVD expansion and spinal column elongation; (2) the use of an adjustable pulley exercise developed to prevent atrophy of spine muscle stabilisers; and (3) other exercises that provide Earth-like annular stress with low-load repetitive active spine rotation movements. The overall objective of these countermeasures is to promote IVD health and to prevent degenerative changes that may lead to HNPs post-flight. In response to “NASA’s Critical Path Roadmap Risks and Questions” regarding disc injury and higher incidence of HNPs after space flight (Integrated Research Plan Gap-B4), future studies will incorporate pre- and post-flight imaging of International Space Station long-duration crew members to investigate mechanisms of lumbar back pain as well as degeneration and damage to spinal structures. Quantitative results on morphological, biochemical, metabolic, and kinematic spinal changes in the lumbar spine may aid further development of countermeasures to prevent lumbar back pain in microgravity and reduce the incidence of HNPs post-flight.

    One purpose of the International Space Station (ISS) is to explore powerful new areas of biomedical science in microgravity. Recent advances in nanotechnology applied to medicine—what we now refer to as nanomedicine—and regenerative medicine have enormous untapped potential for future space and terrestrial medical applications. Novel means for drug delivery and nanoscale screening tools will one day benefit astronauts venturing to Mars and places beyond, while the space laboratory will foster advances in nanotechnologies for diagnostic and therapeutic tools to help our patients here on Earth. Herein we review a series of nanotechnologies and selected regenerative medical approaches and highlight key areas of ongoing and future investigation that will benefit both space and terrestrial medicine. These studies target significant areas of human disease such as osteoporosis, diabetes, radiation injury, and many others.

    This article discusses the development of highly structured membranes for constant release of therapeutics over a broad range of molecular sizes, at release rates relevant for medical applications. In the experimental study, the constant release was achieved with small molecules such as leuprolide, a common treatment for prostate cancer, as well as with large molecules such as bevacizumab, widely used in the treatment of metastatic colon cancer. Such an approach could be applied to achieve the goal of metronomic delivery of chemotherapeutics, a constant low-dose administration of drugs over a long period of time. By exploiting nanochannels in passive systems, researchers were to achieve a controlled and constant delivery for extended periods of time, mimicking the basal and continuous flow of molecules from natural glands. This functionality cannot be attained at the macro- or microscale without the use of complex pumping devices and other moving components, because the diffusion of molecules is Fickian, meaning that the release rate is dictated by the gradient of molecular concentration.

    Traumatic injury strikes unexpectedly among the healthiest members of the human population, and has been an inevitable companion of exploration throughout history. In space flight beyond the Earth's orbit, NASA considers trauma to be the highest level of concern regarding the probable incidence versus impact on mission and health. Because of limited resources, medical care will have to focus on the conditions most likely to occur, as well as those with the most significant impact on the crew and mission. Although the relative risk of disabling injuries is significantly higher than traumatic deaths on earth, either issue would have catastrophic implications during space flight. As a result this review focuses on serious life-threatening injuries during space flight as determined by a NASA consensus conference attended by experts in all aspects of injury and space flight.

    HALS holds promise as a surgical approach in microgravity, particularly as space travel extends beyond low earth orbit. HALS provides the benefits of MIS, facilitates MIS surgery by less surgically proficient or experienced CMOs, and contains equipment and fluid within the operative field. Simulation provides an easy, cost-effective platform to evaluate medical technology for space flight as well as a method to train CMOs on-orbit.

    This disclosure pertains to a system used to entrap clots in cerebral arteries between the artery wall and an external surface of a stent and provide restoration of blood flow therethrough. The system includes a stent forming an annular wall defining a set of openings and an expansion member extendable through the stent to place a distal portion thereof distal to the stent. The stent configured to be advanced into a clot within a vessel and transitioned to an expanded configuration such that the annular wall engages the clot. The expansion member is configured to extend through the stent and to be transitioned to an expanded state such that a diameter of the distal portion is greater than a diameter of a proximal portion thereof. The expanded distal portion is sized to fill a portion of the vessel to prevent clot fragments from flowing distal to the expansion member.
