Value based care becomes most powerful when coordination leads to participation. Connecting the home, primary care, and convenient access creates the infrastructure, while proactive outreach can help patients complete preventive screenings, engage in care programs, use telehealth, and schedule in home assessments. That consistent human connection can help turn an integrated care model into completed care and stronger outcomes.

CVS is doing in healthcare what great retailers learned to do with their customers: build one cohesive view across every touchpoint, then use it to deliver what's relevant. Connecting Signify, Oak Street, and MinuteClinic is what lets that personalization happen at scale, and that's where value-based care starts to deliver on its promise.

Coordination is the hard part and the part that matters most.
