These changes are about removing the tsrm fetch calls, which are extremely
slow. However we have to keep on mind that some of them may not bring a lot
of gains (low usage). But it will the move to TLS when or if it happens.

Also the changes in exts are negligible. I don't buy the #ifdef argument as
every major version brings it lot of #ifdef, be for tsrm or other APIs. And
they occur in the .h of a given ext, one time 5 mins action.
