Noah Breslow shared thisOver the past few years, hospitals have been working through a fragile recovery. Volumes stabilized and COVID-era labor pressures eased. Margins, while still tight, showed signs of improvement in many markets. But something more structural is now taking shape. Costs are rising again. Payer mix is shifting. And demographic trends are changing both the demand for care and how that care is reimbursed. What stood out to me in digging into this is how these forces are compounding. Aging populations, rising uninsured volumes, and growing government payer exposure are all converging inside the revenue cycle, where the operational impact shows up first. That puts a different level of pressure on how health systems manage complexity day to day. I shared a few perspectives on what this shift means and why revenue cycle is becoming core financial infrastructure in this environment. We at Revecore would be interested to hear how others are thinking about this. Personally, I took on a new role in a new industry, with a mission I strongly believe in, and have greatly enjoyed the transition back to operating again. Professionally, our team at Revecore accomplished a ton last year - breaking records, establishing new capabilities, adding key leaders at all levels of the organization, and quickening our pace of execution - all in service of our healthcare provider clients. Among the themes discussed were: 𝗠𝗼𝗮𝘁𝘀 𝗳𝗼𝗿 𝘁𝗲𝗰𝗵𝗻𝗼𝗹𝗼𝗴𝘆 𝗯𝘂𝘀𝗶𝗻𝗲𝘀𝘀𝗲𝘀 𝗮𝗿𝗲 𝗲𝘃𝗼𝗹𝘃𝗶𝗻𝗴. As the cost of building software continues to fall and AI accelerates feature development, traditional sources of defensibility like workflow lock-in are coming under pressure. The focus is shifting from what a product does to the outcomes it delivers. We believe truly proprietary, multi-participant data that powers decisions and outcomes, multi-sided networks, and regulatory complexity are the new defensible terrain. 𝗗𝗮𝘁𝗮 𝗶𝘀 𝗼𝗻𝗹𝘆 𝗮𝘀 𝘃𝗮𝗹𝘂𝗮𝗯𝗹𝗲 𝗮𝘀 𝘁𝗵𝗲 𝗱𝗲𝗰𝗶𝘀𝗶𝗼𝗻𝘀 𝗶𝘁 𝗽𝗼𝘄𝗲𝗿𝘀. The most valuable platforms are those that aggregate operational data across multiple participants and use it to drive mission-critical decisions at scale. We believe this creates a reinforcing flywheel. More proprietary data leads to better models. Better models drive stronger outcomes. Stronger outcomes attract more customers. Over time, this loop becomes increasingly difficult to replicate and forms the foundation of a durable advantage. 𝗧𝗵𝗲 𝗿𝗮𝗰𝗲 𝗶𝘀 𝗻𝗼𝘁 𝗷𝘂𝘀𝘁 𝘁𝗼 𝗮𝗱𝗼𝗽𝘁 𝗔𝗜, 𝗯𝘂𝘁 𝘁𝗼 𝗿𝗲𝗼𝗿𝗴𝗮𝗻𝗶𝘇𝗲 𝗮𝗿𝗼𝘂𝗻𝗱 𝗶𝘁. Our keynote panelists, Ari Balogh, former CTO of Airbnb, and Alex Spinelli, SVP of AI at ARM, were clear: capabilities are already outpacing adoption. In our view, this dynamic creates an opportunity for mid-market companies to close the adoption gap. They are large enough to have meaningful data and a base of customers, but still agile enough to drive change without the implementation challenges inherent in larger enterprises. Nice to see HTD Health's interoperability practice lead Brendan Keeler quoted in Forbes regarding Epic going to bat around health data records. "I talk to at least one group every two weeks that either through ignorance or malice are pushing the boundaries of the network—they’re not pure treatment purposes of use,” says Brendan Keeler, who leads the interoperability practice at HTD Health. “It’s really, really common." Who’s really building the next generation of $1B+ healthcare companies? At SignalFire, we’ve always believed that founder DNA is the strongest signal of future outcomes. In Health & PharmaTech, where complexity, regulation, and distribution hurdles are the norm, that’s especially true. We turned to Beacon, our proprietary data engine tracking 650M+ professionals, to analyze 300+ HealthTech unicorn founders across the country. Here’s what the data showed: - 70% had 10+ years of experience. Execution and trust matter more than hype here - Nearly half came from outside healthcare, bringing fresh playbooks into a legacy industry - 80% had co-founders. HealthTech usually isn’t a solo game - San Francisco had 5x more unicorn founders than Boston, suggesting the center of gravity for HealthTech is shifting west - And the most scalable companies were led by so-called "Centaur Teams" combining GenAI talent with experienced GTM leaders We’re in the early innings of a historic transformation. Something to note from Menlo Ventures's incredible AI in healthcare report....their data confirm what many of us in health systems already feel coming: AI adoption will INCREASE administrative volume before it reduces it. Payers expect, in the next 12–36 months: 63% major jump in call volume 50% more codes per claim 40%+ higher claim volume 38% more prior auths 31% more appeals + resubmissions When documentation gets easier, volume is going to skyrocket. Implications we’re already planning against: - AI adoption doesn’t remove work inherently; it redistributes it across the ecosystem. Workflow redesign is critical. - Providers need guardrails to prevent over-generation of documentation and requests. More noise helps noone. - Startups must show cross-stakeholder ROI, not siloed productivity gains... - Health systems need a system-wide AI playbook, or every local win creates a downstream backlog. This next 2-3 years will either widen payer–provider tension or force real alignment. I'm not counting on the latter. Curious how others are preparing.
