    Positive Observations on Site 1. PPE Compliance: All personnel observed wearing required PPE, including helmets, gloves, and safety boots. 2. Housekeeping: Work areas were clean, organized, and free from obstructions or debris. 3. Working at Height: Workers properly used full-body harnesses and secured anchor points. 4. Tool Safety: Tools were in good condition and used correctly with all safety guards in place. 5. Material Handling: Manual lifting followed correct posture and team lifting protocols. 6. Scaffold Safety: Scaffolds were inspected, tagged, and had complete guardrails and access ladders. 7. Hot Work Safety: Fire blankets and extinguishers were in place; fire watch was actively monitoring. 8. Electrical Safety: Temporary wiring was elevated and protected from damage or water exposure. 9. Lifting Operations: Rigging was proper and supervised, with effective use of hand signals. 10. Signage and Barricading: Hazard signs and barricades were clearly displayed and well-maintained. 11.     Safety First! I came across a striking video today showing a man climbing a ladder over 10 meters high, with an air conditioner on his head, to install it on the third floor of a building. All this, without any visible protection. One wrong step, and it's an assured tragedy. This scene is a brutal reminder of the unacceptable risks taken on some construction sites. It underscores the absolute imperative of a rigorous HSE (Health, Safety, Environment) culture. When faced with such a situation, the first question to ask is: where is the HSE planning? Even before starting work at height, a detailed risk analysis is essential. It is crucial to identify the hazards (falls from height, falling objects, risks related to the load) and to evaluate the associated risks. Next, implementing very rigorous mitigation measures is non-negotiable. For work at height, the use of a mobile elevating work platform (MEWP) with guardrails is the priority collective protection measure. It offers a stable and secure working environment. Let's also not forget the importance of appropriate and duly inspected Personal Protective Equipment (PPE) (helmet, safety shoes, gloves). The mandatory wearing of a safety harness in the MEWP is an essential individual protection measure that enhances safety. This harness must be appropriate for the work and inspected before each use. Finally, the issuance of a work permit acts as a "safety green light," ensuring that all safety conditions are met and verified before the start of the operation. If you die at work, you will be replaced, please make safety at work a priority for your own family's sake. Human life is invaluable. Let's never sacrifice safety for speed or economy.     This incident is a classic example of a "line-of-fire" accident compounded by poor site housekeeping, where the rigger was placed in immediate danger due to proximity to a suspended load. Although a tagline was used, the rigger failed to maintain a safe distance and situational awareness. Key Contributing Factors & Safety Failures Proximity to Suspended Load: The rigger was "too close to the swinging load". Safety protocols mandate that workers never stand under or immediately adjacent to a suspended load to avoid crushing or falling object hazards. Poor Housekeeping (Tripping Hazard): The access path contained accumulated, cluttered, or protruding materials ("heaps of beams"). Lack of Situational Awareness: The rigger moved backward without looking behind them, failing to recognize the hazardous terrain behind them. Improper Tagline Usage: While a tagline was present, it was ineffective because the rigger was not using it to keep a safe distance from the load. A tagline should be used to control the load from a distance, not to guide it while standing next to it. Immediate Actions for Incident Management Stop Work Immediately: The Manitou telehandler operator must stop all movements instantly to prevent the swinging load from striking the fallen worker.     Dear Team, It can be a long way down when you fall from a leading edge. Working from the leading edge can present significant fall hazards if not properly managed. It is critical that all safety protocols and fall protection measures are followed when working near unprotected edges or heights. Here’s how to ensure safety when working from the leading edge.   Before Working from the Leading Edge: Conduct a Hazard Assessment: Evaluate the area to identify any fall hazards. Consider the height, type of work, and potential obstacles. A comprehensive hazard assessment helps determine the necessary fall protection measures. Review your fall protection plan. Use Fall Protection Systems: Ensure that fall protection equipment such as guardrails, safety nets, or personal fall arrest systems (PFAS) is in place and properly secured. If none are available, implement a fall restraint or fall arrest system before beginning work. Train Personnel on Leading Edge Safety: Make sure that all workers are trained in the proper use of fall protection equipment, including how to inspect and maintain the gear. Training on hazard recognition and emergency procedures is also essential. Fall protection 8-hour competent person training.   During Work from the Leading Edge: Wear Proper Personal Protective Equipment (PPE): Always wear required PPE, including a harness and lifeline if working at height. Use helmets, safety boots, and other PPE to reduce the risk of injury from falls or falling objects. Maintain Three Points of Contact: Whenever possible, maintain three points of contact with stable structures, such as ladders or scaffolding. This provides extra stability while working near the edge. Never Work Alone: Always have a partner when working at heights or from the leading edge. Your partner can assist in case of an emergency and help maintain awareness of potential hazards.   After Completing Work from the Leading Edge: Inspect Equipment After Use: After each use, inspect your fall protection equipment for damage or wear. Ensure that it is stored correctly and ready for the next use. Report Hazards or Unsafe Conditions: If you observe any hazards or unsafe conditions while working from the leading edge, report them immediately to your supervisor. Immediate action prevents potential accidents in the future. Review Work Procedures: After completing the job, review safety procedures with the team to discuss what went well and what could be improved for future projects. Continuous safety improvement is vital for ensuring a safe work environment. Working from the leading edge requires awareness and preparation.
