Even for a software company, this pace would be very impressive, but Neil Sobin is doing it with hardware, out at sea, in incredibly volatile geopolitical conditions. Neil started Quartermaster with a simple premise: the only sensing network that can actually scale to the ocean is the one that's already on the ocean. This team is moving quickly, putting their SmartMast product on 650 working vessels from 25 countries. It very cleverly combines HD video, AI, radar, RF sensing, and AIS to turn working vessels already on the water into a live sensor network for the ocean’s surface. This increased visibility helps mariners of all types navigate congested waters and busy ports, avoid potential hazards, and call for help in an emergency. It also comes with tremendous built-in network effects that only become more valuable as they scale up. Neil has gone unreasonably deep on how all these different pieces of the business fit together, and we are so fortunate to be his partner here First Round Capital.

Josh Kopelman shared thisWhen Marta Bralic Kerns pitched us for Pomelo Care’s seed round back in 2021, there was a thesis behind the name: a pomelo has a thick rind that protects what’s inside. More than 5 years later, the team is now sharing a new look and a growing mission, but that founding metaphor is still central. Pomelo is expanding from their roots in maternal health to provide care that’s one step ahead for women and children across every stage of life, including pediatric care and (peri)menopause. The idea is to wrap a continuous layer of care around a family in order to catch small health changes before they become complications. Couldn’t be more proud of Marta and all the progress she and her team have made.

Big win for the Inito team on closing a $29M Series B led by Bertelsmann India Investments - a major milestone for a company redefining at-home health diagnostics and using AI-designed antibodies to unlock entirely new kinds of tests. We backed Inito early as one of our first investments out of Fund I, and have watched the company grow from a bold idea into a platform built around making quantitative fertility hormone testing possible at home, globally. By measuring estrogen, LH, PdG, and FSH on a single test strip - and applying AI to interpret hormone patterns - Inito delivers a level of clarity and confirmation that traditional ovulation tests simply don’t provide. Since launching over five years ago, Inito has analyzed more than 30 million hormone data points and supported tens of thousands of users - proof that care can be more precise, more personal, and more empowering when it starts at home. Excited to see the team build on this momentum as they expand beyond fertility into a broader at-home diagnostics platform and continue pushing healthcare toward a more proactive, data-driven future. Orange Health Labs has always been committed to six-hour reporting. Not "as fast as possible" but specifically six hours, no exceptions. This one constraint made them build everything differently. They couldn't use standard labs designed for average daily volume - they had to build for peak hourly capacity. They couldn't have doctors at each location, so they built remote pathology, where one doctor reviews slides from multiple cities. They couldn't rely on traditional logistics - so they created dedicated networks covering four times the area of competitors. Now incumbents can't copy it without scrapping their existing infrastructure. They have hundreds of labs built the old way, doctors hired locally, and established logistics. Retrofitting would cost more than starting from scratch, and starting from scratch means abandoning their existing business. I like how Orange Health's edge is that matching their model means incumbents must treat their current infrastructure as sunk cost. This is the kind of advantage that compounds.

At Bread and Butter Ventures, we're in our 7th year of investing in digital health. I've been hearing a lot of feedback lately from both funds and founders alike that VC funding for tech-enabled care delivery is fewer and far between these days. I'm super bullish on tech-enabled care as a component of a digital health portfolio because I believe that true, lasting innovation will be driven by bottoms-up early stage innovation responding to top-down industry pain points, and that to innovate we must have distribution through the incumbent plumbing of the healthcare system - and that includes care at the center. Yes, the margins are lower than pure software, but the distribution network of healthcare is very different than pure B2B. We're super interested right now in: ✅ Applications of AI to in infrastructure and operations of health care systems, provider networks, health plans to reduce manual work and enable everyone to operate at their best and highest use ✅ Applications of AI to clinical decision support and ultimately care delivery ✅ Solutions that address population health and help with access to care in both rural and urban settings (both from a care & cost perpspective), cardiometabolic health, women's health (still wildly uninvested opportunities to serve half the population), eldercare ✅ Insurtech, fintech and the payments layer of healthcare We would love to connect with more co-investors at all stages (pre-seed, seed, later stage) who are actively investing in digital health and specifically at tech-enabled care solutions to drive the next generation of healthcare innovation. Some quick and candid / not just bullish observations on AI in digital health, especially given the enormous level of noise at #JPM2026. Last (5th) post in the 5-on-5 series, opening 2026 with 5 different pieces with 5 observations each on the market for digital health founders. We are passed the hype phase. I actually think 2026 will be the year when saying 'AI' or 'Agentic' will raise more questions than applause. Too many pilots are not scaling, too many promises are yet to be fulfilled. When building a product takes less time, the path to commoditization also takes less time. Scribes, chart summarizations, etc. which leads IMO to NRR being much more important than ARR (show you are resilient to price pressure and churn) Most real impact of AI is on the back end of new companies. This is massive and hence any budget that looks similar to 2023 / 2024 or even 2025 budgets should render a very serious question As a byproduct, and unlike 2021, increased head count becomes a negative signal if the company is indeed AI-centered (proxy to inefficiency) And another key difference from 2021: raising a lot of money (with a big valuation) while the public market is at best open for high 9-digit revenue, is really playing very aggressive poker.
